At 24 May 1925 is founded first football club in Sandanski - Ustrem. The first matches of the new club are versus football club Lubomir Vessov from Petrich and are won from Ustrem Sandanski. In 1938 FC Ustrem won his first prize at small tournament with teams from Petrich and Gorna Djumaya.

In 1957 there is reorganization of all of sport clubs in Sandanski and is created new united sport club - Vihren. To the season 1976/1977 the club is in 3rd Bulgarian division (South-Weastern "V" RFG) when won right to get to upper division ("B" group). In season 1979/1980 the team is second in "B" group after the team of Belasitsa Petrich.
Season 1988/1989 Vihren Sandanski fall again to the lower 3 division and after this the team pass through 2nd and 3rd divisions periodically.
The best season for Vihren is 2004/2005 when the team won 1st place in second football division and get to Bulgarian High Division (A group). The team is sensation in first stages of Bulgarian Football Championship.
